Your First Visit

Welcome to the Union City Dental Care Center. We look forward to being of service to you. Our clinic is a component of the University of the Pacific, Arthur A. Dugoni School of Dentistry. Our Resident Dentists work under the supervision of the dental school faculty, many of whom are specialty trained.

If this is your first visit to our clinic, we will ask you to provide us with information regarding your past and present medical history. Please download and complete the medical history form. This information is important for us to be able to care for your dental health needs safely, so please be as complete and accurate as you can when providing this information. For patients with special medical conditions, it's highly advisable to bring your current medical laboratory results with you as well as a list of all the medications you are currently taking. In some instances, it may be necessary to contact your physician to clarify questions we may have regarding your health status. We do this for your safety and well being.

If you routinely take antibiotic prophylaxis prior to dental appointments, please take them as prescribed before coming. If you are unsure if you need antibiotic prophylaxis, you should still come for your first appointment, but please do be aware that we may not be able to complete the entire comprehensive examination on that day.

If you have been referred to our clinic by another office it can be very helpful to bring a referral note with the dentist's name and phone number. In some instances we may be able to use current x-rays from other offices, however; it may be necessary for us to make additional x-ray images of your teeth in order to properly identify any dental problems you may have.

Please give yourself plenty of time to travel here on your first visit, especially if you are unfamiliar with the area. Parking is available in our parking lots and on the street in front of the clinic building.
